The process or returning product to Capital Returns is very simple:
- Request labels: Capital Returns program includes postage-paid UPS ARS (Authorized Return Service) labels. If you need labels, please feel free to fill in the form to the right, or contact a Capital Returns service representative today.
- Form 222: If shipping and Schedule II Controlled Substances, you must request a form 222 from Capital Returns. You may either fill in the "222 Request" form to the right, or again, you may contact a Capital Returns service representative to assist you in this process.
- Pack: Once you are ready to ship your product, please line a sturdy cardboard box with a plastic bag (but not a biohaz bag), place a layer of packing material on the bottom of the box to minimize breakage in shipping, and then carefully place the items in the box. Please ensure that any Schedule II Controlled Substances are placed in the free tamper-evident bags that Capital Returns provides, and that this bag is placed at the top of the box. Likewise, please ensure that any Schedule III-V Controlled Substances are placed in their own bags, again making sure these are placed at the top of the box.
- Ship product: Ship the product to Capital Returns at 6101 North 64th Street, Milwaukee, WI 53218 using our postage paid UPS ARS labels.
The returns program is that simple: just pack and ship. If time is a factor, Capital Returns can assist with our On-site service, where we send one of our representatives to your facility to assist in the inventorying and shipping of the product. This program is especially beneficial to pharmacies located in Ohio & Florida where regulations require that ALL products be inventoried prior to shipment.
